[ARCHIVED] Community Health Needs Assessment Survey

Every 3 years, the Wilkes Health and Wake Forest Baptist Health-Wilkes Medical Center complete a Community Health Needs Assessment (CHNA). This is a comprehensive look at the overall health of Wilkes County. The assessment uses state and local data as well as a community behavior survey to help identify strengths, weaknesses and help identify priority areas over the next few years. Not only does Wilkes Health use the data, but Wake Forest Baptist Health - Wilkes Regional Medical Center, the Health Foundation, and other community agencies use this information to understand community health needs also. Please take a few minutes to complete the survey. You must be at least 18 or older and live in Wilkes County. If you have already taken the survey on paper or online, please do not take it again. The goal is to have the survey open for approximately 4 weeks.

The answers that you provide in the survey will not be linked back to you in any way.

If you wish to participate, there is a chance to win one of several cards, please continue to the end of the survey where you can find out how to enter the drawing.

The link for the survey is here.
