To apply for an absentee ballot by mail, send a NC State Absentee Ballot Request Form to

The form can be mailed, emailed, faxed or brought in person to our office by the voter, near relative or verifiable legal guardian.

Upon receipt of your request, the county board of elections will send you an absentee ballot and instructions for completing the ballot. You must submit your request no later than the Tuesday prior to Election Day. The request may be made by the voter, a near relative or verifiable legal guardian.
